Medication Management

1. Help the client organize their prescription regimen
2. Communicate with the Prescriber and/or Pharmacist to confirm medication regimen
3. Discuss client referral possibilities to other services as needed
4. Help client manage their care schedule.

RxHSM is not a pharmacy and is not licensed to dispense prescription medication. Client's questions about medication interactions, dosage, frequency of dosage, and other medical questions related to each medication should be directed to the Prescriber and/or the Pharmacist. The client should understand that RxHSM is not a medical practice overseen by a physician and may not diagnose medical conditions or prescribe medication. The client should understand that in the event of a medical emergency he or she should call 911 and seek emergency medical care.
